The instrument(s) from MIS Quarterly have been reproduced both with the permission of the authors as well as the editor (Robert Zmud) and managing editor (Susan Scanlan) of this journal. The instrument(s) from Information Systems Research have been reproduced with the permission of the authors as well as the editor (John King) of this journal. Deborah Cotton (Copyright & Permissions) has given us permission to reprint surveys from Association for Computing Machinery journals. Angelo DeNisi, the editor of the Academy of Management Journal, has also told Jane Webster there is no problem with her providing a link to her instrument from this journal. The instrument in Management Science devised by James Bailey and Sam Pearson has been reproduced with the permission of the authors and Candi Gerzevitz of INFORMS.
